Facts in brief:
The victim, a process worker, was tasked with operating a Dirinler Eccentric 60 tonne press. The victim was unaware that the machine had been changed from single stroke mode to continuous stroke mode. After the first mould was pressed, the victim reached into the lower die with her right hand to retrieve the excess metal. Due to the lack of guarding on the machine, when the press cycled again, it came back down on her thumb causing serious injury. The key enabling the modes to be changed was left in the machine.
