Notifications  
If someone has been seriously injured, become seriously ill, or died as a result of work – phone us on 0800 030 040 straight away. We have staff available to respond to these 24/7.

Notifications made through our online form won't be monitored between 12pm on Tuesday 24 December 2024 and 8.30am on Monday 6 January 2025.
If you’re not sure if you need to notify us, use our online notification system and we’ll respond to you after 6 January 2025.

General enquiries
General enquiries made by phone or email after 12pm on Tuesday 24 December will be responded to from Monday 6 January 2025. This does not apply to notifications made by phone on 0800 030 040.

Operators of facilities or proposed facilities are required to notify WorkSafe of specified hazardous substances that are present, or likely to be present.
The good practice guidelines Major Hazard Facilities: Notifications and Designation will assist you to complete the notification form and explains the notification and designation process.
Use the Notification of Major Hazard Facility form to notify WorkSafe of your facility.
Use the MHF calculator to assist you with calculating the quantities of specified hazardous substances and whether your facility is either an upper tier major hazard facility, lower tier major hazard facility, or neither. You may choose to submit a copy of the MHF calculator along with your notification form.
Design notices
The good practice guidelines Major Hazard Facilities: Notifications and Designation offers advice on how to submit a design notice. There is no prescribed form for a design notices but it must include the matters in Schedule 6 of the regulations.
