On 12 February 2023, an ITCL worker operating as a Traffic Controller was struck by a truck while operating a temporary roadblock. The worker died as a result of his injuries.

On 27 May 2023, a 9-axle truck and trailer unit fully loaded with logs was negotiating a corner on a private road within a wood lot harvest operation. As the truck rounded the corner, it left the road and rolled down a bank, causing the driver of the truck to sustain fatal injuries.

On 13 September 2023, in the truck bay area of the Papamoa Pak’n Save supermarket operated by PAPPNS, a reversing forklift collided with a worker who sustained serious foot injuries.

On 7 December 2023, the Kura took a group of students and other children on a river float activity on the Waikato River in Taupō. They were accompanied by one teacher and one teaching assistant and were using pool floatie toys and pool noodles to float down the river. The group got into trouble and needed to be rescued.

On 27 February 2023, a Claymark worker sustained serious hand injuries, including amputations of two fingers, after making contact with the moving parts of an industrial wood planer machine.

On 7 March 2023, an Ecostore worker sustained chemical burn injuries when a pressurised hose carrying a hazardous liquid disengaged from its clamp, spraying the substance into the air and contacting the worker’s eyes.

On 25 July 2022, a worker at the University of Waikato's campus was changing a compressed industrial cylinder containing high-purity oxygen. When the cylinder was turned on, an explosion occurred, resulting in a fireball. The worker, who was in close proximity, was engulfed in the blast and sustained serious injuries.

On 3 May 2022, a worker was inside the guarded perimeter fence of a mesh welding machine assisting with fault finding and minor repairs when the machine restarted, trapping him between the conveyor bars and causing serious impact injuries.

On 4 January 2022, two workers were undertaking a pre-start sanitisation clean of the processing area and equipment at SFF’s Hokitika processing plant when they inhaled a hazardous substance, resulting in hospitalisation.

On 24 March 2022, while driving down the carriageway to exit the education provider's site, the victim's car collided with an unsecured steel boom gate that had been blown into the carriageway by a gust of wind, resulting in severe injuries.

On 25 April 2022, while working as a forklift driver at Trevelyan’s site in Te Puke, the victim was struck by another forklift while walking behind it as it was reversing. The incident caused severe injuries to his lower left leg, leading to amputation below the knee.

On 7 July 2021, during commissioning activities at Rotokawa Power Station, fluids with significantly different temperatures unintentionally mixed, resulting in a severe steam hammer event that damaged valves and pipework, leading to the loss of steam containment. No injuries were sustained.

On 20 December 2021, while operating a docking saw and trimming ragged ends of timber, a worker suffered serious injuries to his hand when he was struck by the saw’s blade.

On 30 August 2021, a member of the public collided with the rear of a stationary Police mobile speed camera vehicle on the Upper Harbour Motorway near Greenhithe. The collision resulted in the death of the motorist and caused serious injuries to the Police worker who was thrown from the vehicle.

On 15 November 2021, while clearing geese from the airport runway, a pyrotechnic cartridge ignited in the victim's hand causing serious injuries.

On 7 December 2020, two students and a worker were trapped in a sea cave on an excursion to the Poor Knights Marine Reserve. All suffered cuts and abrasions; the children suffered emotional trauma.
