If someone has been seriously injured, become seriously ill, or died as a result of work – phone us on 0800 030 040 straight away. We have staff available to respond to these 24/7.

Notifications made through our online form won't be monitored between 12pm on Tuesday 24 December 2024 and 8.30am on Monday 6 January 2025.
If you’re not sure if you need to notify us, use our online notification system and we’ll respond to you after 6 January 2025.

- To have provided up to date and model specific manual/instructions for the converted Swing Loader, particularly covering the use of the remote control unit, at the time of supply following the conversion in 2011 and the repairs in 2014;
- To have provided accurate electrical drawings as part of the updated manual/instructions;
- To have supplied the Swing Loader with accurate signage indicating safe zones, weight loads and markings on the manual levers.
- To have ensured that any testing and certification of the Swing Loader was documented and provided to the purchaser of the Swing Loader.
